Sarcopenia is caused by a combination of sociodemographic, behavioral, physiological, and climatic factors, underscoring the relevance of studying predictors of muscle strength decline in populations living in unfavorable natural and climatic conditions of Russia’s European North.</p> <p><bold>AIM</bold><bold>:</bold> The work aimed to assess the influence of sociodemographic characteristics, lifestyle factors, anthropometric measures, and body composition parameters on handgrip strength in middle-aged and older adults residing in the Russian Arctic.</p> <p><bold>METHODS</bold><bold>:</bold><bold> </bold>The study used data from two cross-sectional population-based studies conducted 7.2 years apart in a single random sample of the adult population of Arkhangelsk (n = 1168). Associations between handgrip strength and sociodemographic characteristics, lifestyle factors, anthropometric measures, and body composition parameters were assessed cross-sectionally and longitudinally using linear and logistic regression analysis.</p> <p><bold>RESULTS</bold><bold>:</bold><bold> </bold>Cross-sectional analyses revealed an age-related decline in handgrip strength in both men and women, with a greater reduction in men. Handgrip strength was positively associated with height, body mass index, waist circumference, and waist-to-height ratio, as well as with higher education in men. Lower handgrip strength was associated with smoking in men and financial difficulties in women. According to bioimpedance data, higher basal metabolic rate and fat mass percentage were associated with greater handgrip strength, whereas higher muscle mass percentage and body impedance were linked to lower strength. Patterns observed cross-sectionally were partially confirmed in longitudinal data. Greater height, general and abdominal obesity parameters, and higher basal metabolic rate were associated with a lower risk of decline in handgrip strength over time in women, whereas older age, greater body impedance, and higher proportions of muscle mass and body water increased the risk. In men, older age was associated with a higher risk of strength decline, whereas greater height reduced the risk.</p> <p><bold>CONCLUSION</bold><bold>:</bold> Handgrip strength in middle-aged and older adults was determined by a set of sex-specific factors. Longitudinal decline in handgrip strength was more strongly associated with anthropometric and body composition parameters than with sociodemographic or behavioral factors. These findings highlight the importance of considering sex-specific differences when developing sarcopenia prevention strategies for residents of the Russian Arctic.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Снижение мышечной силы с возрастом связано с изменениями метаболизма и оказывает существенное влияние на функциональное состояние и качество жизни пожилых людей. Развитие саркопении определяется совокупностью социально-демографических, поведенческих, физиологических и климатических факторов, что обусловливает актуальность изучения предикторов снижения мышечной силы у населения, проживающего в неблагоприятных природно-климатических условиях Европейского Севера России.</p> <p><bold>Цель.</bold> Оценка влияния социально-демографических характеристик, факторов образа жизни, показателей антропометрии и состава тела на силу кистевого хвата у лиц среднего и пожилого возраста, проживающих в Арктической зоне Российской Федерации.</p> <p><bold>Методы.</bold> Исследование основано на использовании данных двух поперечных популяционных исследований, проведённых с интервалом 7,2 года на одной случайной выборке взрослого населения Архангельска (<italic>n</italic>=1168). Оценка связи между силой кистевого хвата и социально-демографическими характеристиками, факторами образа жизни, антропометрическими показателями и параметрами состава тела выполнена в поперечных срезах и на лонгитюдных данных с использованием линейного и логистического регрессионного анализа.</p> <p><bold>Результаты.</bold> В поперечных срезах снижение силы кистевого хвата с возрастом наблюдалось у мужчин и женщин, более выраженным было у мужчин. Сила кистевого хвата была положительно связана с ростом, индексом массы тела, окружностью талии и отношением окружности талии к росту, с наличием высшего образования у мужчин. С более низкими значениями силы кистевого хвата были связаны курение у мужчин и финансовые трудности у женщин. По данным биоимпедансометрии, более высокая скорость основного обмена и более высокая доля жировой массы ассоциированы с большей силой кистевого хвата, тогда как более высокая доля мышечной массы и больший импеданс тела — с меньшей силой. Закономерности, наблюдаемые в поперечных срезах, частично подтвердились на лонгитюдных данных. С меньшими шансами снижения силы кистевого хвата в динамике у женщин были связаны более высокий рост, показатели общего и абдоминального ожирения и более высокая скорость основного обмена, с повышением шансов — возраст, более высокий импеданс тела, большие процентные доли мышечной массы и воды в составе тела. У мужчин с большими шансами снижения силы кистевого хвата в динамике был связан более старший возраст, с меньшими шансами — более высокий рост.</p> <p><bold>Заключение.</bold> Сила кистевого хвата у лиц среднего и пожилого возраста определялась совокупностью факторов, значимость которых варьировала в зависимости от пола. Снижение силы кистевого хвата в динамике в большей степени ассоциировано с показателями антропометрии и состава тела, чем с социально-демографическими и поведенческими факторами.
