Respiratory diseases occupy a leading place in the structure of childhood morbidity and hospitalization. A long-term analysis of the dynamics and structure of hospitalized morbidity is necessary to develop targeted prevention programs.</p> <p>AIM: To study the dynamics, structure, and clinical and demographic characteristics of the hospitalized incidence of respiratory diseases in children and adolescents according to the data of a multidisciplinary third-level children's hospital for 2010-2024.</p> <p>METHODS: An observational, single-center, retrospective, selective, and uncontrolled study was conducted. The research was based on the multidisciplinary children's hospital of the Russian Ministry of Health. The study included cases with a verified primary diagnosis at discharge, coded according to ICD-10 in class J00–J99 (respiratory diseases). The study period was 15 years (from 2010 to 2024). The criteria for exclusion from the study were the incompleteness of cases (2025), as well as the lack of verification of the diagnosis or the absence of the ICD code. Statistical data processing was carried out using the Mann–Whitney, Kraskel–Wallis, Pearson chi-square, Spearman correlation criteria.</p> <p>RESULTS: In 2010-2024, 39,824 cases of hospitalization with diagnoses of class J00–J99 were registered. Of these, 18,178 cases (45.6%) were in the pulmonology department (PD2), 21,646 cases (54.4%) were in the infectious diseases department (ID1). The number of hospitalizations ranged from 2,636 cases in 2010 to a maximum of 3,249 in 2016. In 2020, the minimum number of hospitalizations was 1,721 cases, due to restrictive measures during the COVID-19 pandemic and the conversion of the bed stock. In 2024, the number of cases was 2,123.</p> <p>CONCLUSION: The nosological profile of pediatric hospitalizations for respiratory diseases has undergone significant changes in the direction of chronic pathology. The duration of hospitalization significantly depends on the nosology, age, gender, and form of medical care. The data obtained substantiate the need to develop differentiated standards for the duration of inpatient treatment and targeted preventive strategies.</p>
<trans-abstract xml:lang="ru"><p>Обоснование. Полученные данные обосновывают необходимость разработки дифференцированных нормативов длительности стационарного лечения и целевых профилактических стратегий.</p></trans-abstract><trans-abstract xml:lang="zh"><p/></trans-abstract><kwd-group xml:lang="en"><kwd>respiratory diseases</kwd><kwd>children</kwd><kwd>adolescents</kwd><kwd>hospitalized morbidity</kwd><kwd>bronchial asthma</kwd><kwd>ARVI</kwd><kwd>pneumonia</kwd><kwd>length of stay</kwd><kwd>epidemiology.</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>болезни органов дыхания</kwd><kwd>дети</kwd><kwd>подростки</kwd><kwd>госпитализированная заболеваемость</kwd><kwd>бронхиальная астма</kwd><kwd>ОРВИ</kwd><kwd>пневмония</kwd><kwd>длительность госпитализации</kwd><kwd>эпидемиология.</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>1.
