THE SOCIAL HYGIENIC FACTORS OF QUALITY OF LIFE OF POPULATION OF THE PRIMORSKY KRAI
Abstract
At the modern stage of society development, even more number of professionals of the most various specialties show interest in problems of estimating level and quality of life of population. The study was carried out to analyze main indices of quality of life of population of the Primorsky Krai. The study used statistical information characterizing quality of life of population (indices of issues of demography, ecology, labor, level of life of population, education, medical and social care). The analysis of quality of life of population of the Primorsky Krai during 2004-2013 according indicator of "crisis" index testifies that a trend of increasing of level of life of population is observed. The complex evaluation of quality of life using factor analysis demonstrated that 81.7% of total variance is described by by main component. In main component prevail variables of economic and social character, demographic indices and also indices of population health, housing conditions and ecological environment. The main component proved to be very significant in characterizing quality of life of population of the Primorsky Krai. The obtained results demonstrate that factually all selected factors in a certain degree effect quality of life of population that one more time proves all multiform and complexity of quality of life concept. Therefore, quality of life is a complex characteristic of conditions of vital activity of population that is reflected in objective indices and subjective estimates of satisfaction of material, social, cultural needs and is related to perception of people of one's own positioning depending on cultural features, system of values and social standards existing in society.

THE INCREASING OF MORTALITY IN THE REPUBLIC OF CRIMEA: SCALE, CAUSES, PROBLEMS OF PREVENTION
Abstract
In 2014, increasing of mortality is observed in the Republic of Crimea. The attempts to normalize situation using common preventive means turned out to be not enough effective. The purpose of study is to analyze causes of deterioration of demographic situation and to substantiate measures of efficacious prevention. The data of state statistics was used as a primary material. The mortality of main non-infectious diseases prevailed in the structure of human losses. However, its dependence on classical risk factors had no factual confirmation. The ecological situation ameliorated. The health care system maintained a needed stability. The deterioration of psychic condition of population was detected. The suggestion was expressed concerning its effect on somatic reserves of health. In conditions of etiological uncertainty of mortality decreasing a personalized prevention was proposed targeting management of health reserves under control of personal risks. The percentage of Crimea population with redundant lethal risk makes up to no more than 5%. The modernization of actual dispensarization system is proposed to detecting and health improving them. The working population is proposed to pass screening examination without breaking labor process on the basis of health posts (health room) of enterprises. The traveling diagnostic teams can be used. The idle population is examined in the departments of prevention in polyclinics. The low cost prognostic technologies are used. The selection of individually efficient health promoting measures requires long work with patient. to implement this approach a third stage of dispensarization is proposed using innovative health promoting technologies. The medical social workers are involved into this activity. The trade union organizations can play an important role in the prevention system functioning. The particular attention is paid to using health promoting possibilities of recreation institution of Crimea.

THE EFFICIENCY OF MANAGEMENT BY OBJECTIVES PLANNING TO SUPPORT SANITARY EPIDEMIOLOGICAL WELL-BEING OF POPULATION AND DEFENSE OF RIGHTS OF CONSUMERS IN THE FEDERAL SUBJECT
Abstract
The article analyzes efficiency of management by objectives planning in supporting sanitary epidemiological well-being of population and defense of rights of consumers in the subject of the Russian Federation. Within the framework of this analysis a characteristic is given concerning supporting sanitary epidemiological well-being of population and defense of rights of consumers according sectoral target programs in the subject of the Russian Federation. The results of analysis of social economic efficiency of management by objectives are presented. The sociological survey is implemented concerning issues of quality and accessibility of public services provided by Rospotrebnadzor and effective management of sanitary epidemiological well-being of population. The originally elaborated technique of evaluation of efficiency of management by objectives is presented within the framework of achieving strategic targets of Rospotrebnadzor in the subject of the Russian Federation.

THE CHARACTERISTICS OF ONCO-UROLOGICAL MORBIDITY OF MOSCOW POPULATION IN 2006-2015
Abstract
The article presents the analysis of trends specific for onco-urological morbidity of Moscow population on the basis of data of official statistical reports for 2006-2015. It is established that prostate cancer is the most actual onco-urological problem in Moscow. The level of morbidity of prostate cancer in the capital exceeds the corresponding average level in the Russian Federation. During 2006-2015, the morbidity of prostate cancer in Moscow increased up to 33.6% i.e. from 52.7 to 70.4 cases per 100 000 of male population. In Moscow, in 2015, morbidity of kidney cancer made up to 11.73 and bladder cancer - 9.72 cases per 100 000 of population. As compared with 2006, morbidity of prostate cancer decreased up to 3.1% and morbidity of bladder cancer increased up to 9.2%. In Moscow, for morbidity of prostate cancer and bladder cancer are specific lower levels in comparison with national average indices. It is demonstrated that efficiency of target oncologic preventive examinations, timeliness of detection of onco-urological diseases and also efficiency of their treatment in Moscow is higher than in the Russian Federation on the whole. At that, in Moscow 31.6% of cases of prostate cancer, 29,7% of cases of kidney cancer and 17.7% of bladder cancer are detected at stage III-IV of disease. The results of implemented study testify necessity of development of urological care of population, including its out-patient section that effects efficiency and timeliness of detection and effectiveness of treatment of onco-urological diseases.

THE ALGORITHM OF PROVIDING MEDICAL PREVENTIVE CARE OF PATIENTS OF RISK GROUP OF DEVELOPMENT OF MALIGNANT TUMORS OF SKIN
Abstract
The malignant skin tumors are neoplasms of visual localization and their diagnostic is extremely complicated because of diversity of their clinical forms. Among all skin malignant tumors, a melanoma holds a particular place being socially important problem because of high level of lethality related to significant metastatic potential of tumor and lower efficiency of therapy of later forms of disease. The article describes development and implementation of algorithm of rendering medical preventive care to patients of risk group of development of malignant skin tumor on the basis of investigation of actual epidemiological situation with morbidity of basalioma, melanoma, squamous cell carcinoma of skin in population of the Sverdlovsk region during 2000-2015 and detection of factors conditioning late diagnostic of malignant skin tumors. According to actual epidemiological data, an increasing of morbidity of both melanoma and other malignant skin tumors is observed in the Russian Federation on the whole and in the Sverdlovsk region. The article pays attention to that physicians of various specialties are to timely send patients of risk groups or with «suspicious» neoplasms to dermatovenerologist to specify character of neoplasm and to resolve issue concerning necessity of dispensary observation. The necessity is noted of tighter interaction of dermatoverologists and oncologists. Thereby, implementation of algorithm of rendering medical care to patients of risk groups of malignant skin tumors in the Sverdlovsk region promotes increasing of quality and accessibility of medical care to population, earlier detection of patients, decreasing of load of oncologic service by non-profile patients.

THE REGIONAL APPROACH TO EVALUATION OF CONDITION OF UNSPECIFIED RESISTANCE OF ORGANISM IN CHILDREN POPULATION
Abstract
In Lipetsk region during 9 years a monitoring was implemented concerning immune status of children of preschool age residing in conditions of multi-factorial impact of environment. The sampling included 8,640 children. The examination implemented non-invasive screening techniques analyzing bactericidal activity of skin and composition of skin microflora. The children were allocated in three groups according their health: group I - healthy children; group II - children with functional and certain morphologic divergences and also decreased resistance to acute and chronic diseases; group III - children with chronic diseases at the stage of compensation with preserved functional possibilities of organism. The results of study demonstrated that bactericidal activity of skin within the limits of standard was established in 68% of children from group I, 65% of children from group II and 58% of children from group III. In the rest of children alterations were established related to composition of viable resident microflora being accompanied by increasing of index of bactericidal activity of skin or its drastic decreasing up to 37-58% that testifies disorder of adaptation compensation reactions of organisms of children to conditions of impact of unfavorable environment factors. In 10.4% of children from group III study established significant damage of structure of microbiocenosis of skin and Schneiderian membrane in direction of increasing of number of microorganisms with signs of pathogenicity occurring more often in boys than in girls that substantiates disorder of cellular immunity of organism in the given group of children. These children are included into the risk group for in-depth dispensary observation.

THE INDICES OF MORBIDITY OF PANCREAS CANCER IN WORKERS OF RADIATION DANGEROUS ENTERPRISE
Abstract
In Russia, during last decades increasing of morbidity of pancreas cancer is observed. This disease is among leading causes in the structure of mortality of malignant neoplasms. The data concerning effect of ionizing irradiation on development of pancreas cancer is ambiguous. The analysis was carried out concerning indices of morbidity of pancreas cancer in cohort of workers for the first time hired to enterprise of nuclear industry production association «Mayak» in 1948-1982 exposed to prolonged total external gamma-irradiation and internal alpha-irradiation by incorporated plutonium. It is established that in both considered cohort and total population, indices of morbidity of pancreas cancer depended on such non-radiation factors as attained age, gender, calendar period of observation, smoking and value of smoking index, alcohol abuse. The study established increasing of standardized by age indices of morbidity of pancreas cancer in workers exposed to prolonged external gamma-irradiation in doses 0.5 Gy and higher. The effect of internal alpha-irradiation on indices of morbidity of pancreas cancer was absent in examined cohort.

THE EVALUATION OF NOISE EFFECT ON BIOCHEMICAL INDICES OF ENDOTHELIAL DYSFUNCTION IN WORKERS OF SELWYNITE ORE-DRESSING AND PROCESSING ENTERPRISE
Abstract
The labor conditions of workers of key professions of selwynite concentrating manufacture (drying machine operative, granulation machine operative, dosing of drying-granulating room machine operative, centrifuge operative, filtration operative, transporter of drying granulating room, engineer of mill machine) are characterized by effect of physical factors of industrial environment. The noise is the most significant of them. It is established that level of noise at working places of workers of analyzed specialties attains 83-98 dBA that on 18 dBA is higher of maximum permissible level of 80 dBA. In examined workers a reliable increasing of level of homocysteine and lipoprotein (a) up to 3.8 times is established against the background of decreasing of production of nitric oxide in blood serum up to 1.2 times and increasing of level of malonic di-aldehyde up to1.3 times relatively to similar indices in workers of comparison group. The consequence of these processes can be a disorder of structure and endothelial function of vessels, increasing of permeability of endothelium, loss of its elasticity and as a consequence disorder of functional activity of vessel wall. The substantiation is given to indicator indexes (level of lipoprotein (a), homocysteine in blood serum and content of malonic di-aldehyde in blood plasma), having higher and average degree of occupational conditionality and forming biochemical profile of endothelial dysfunction as a predictor of arterial hypertension in workers of selwynite ore-dressing and processing enterprise undergoing effect of noise. The given biochemical indices are appropriate to be applied in early diagnostic and prevention of endothelial dysfunction in workers of the mentioned enterprises.
