APPLICATION OF GENETIC CRITERIA IN THE OCCUPATIONAL SELECTION OF LABOUR RESOURCES FOR THE OIL FIELDS OF SIBERIA

Abstract

225 men employed in the oil industry of the Tomsk and Tyumen Regions of Siberia, the Russian Federation, have been examined. It has been established that the frequency of micronucleated buccal epithelial cells and concentrations of urino-excreted benzapyrene in the workers were significantly higher then in the control group. The highest levels of the indices, including the increased frequency of cells with a large micronucleus (more than 3 pm 2 in diameter), were observed in the individuals with mutant alleles of GSTM1 gene. There has been established a correlation between the frequency of micronucleated cells and the concentrations of benzapyrene in urine. The activation of mutation process in the oil industry workers is presumed to be a result of combined effects of various factors on the genome including not only strong mutagens, but also co-mutagens. It should become a base for implementation of the new, scientifically based criteria for hiring personnel for oil industry works in Siberia.

STATE OF COGNITIVE FUNCTIONS IN ABLE-BODIED MEN DRIVING MOTOR TRANSPORT

Abstract

We have assessed cognitive functions in 103 able-bodied men (66 men were drivers; 37 men were in the control group, their profession was not connected with vibration). In each group, healthy patients and patients with arterial hypertension (AH) were separated. The cognitive functions were studied with use of the Montreal Cognitive Assessment Scale (MoCA-test). Cognitive disorders were found in 45.5 % of the drivers and in 29.7 % of the men from the control group. A correlation between the level of the MoCA-test index and the profession of the drivers has been found. Statistically significant between-group differences in the state of delayed memory, attention, orientation, verbal fluency of the drivers and the control group (with the significance level of p < 0.05) have been found.

DIFFERENT VARIANTS OF CARBOHYDRATE METABOLISM DISORDER AND ITS INFLUENCE ON MYOCARDIAL INFARCTION COURSE

Abstract

Altogether 360 patients (63.4 % of men) were examined with the aim to reveal the incidence of carbohydrate metabolism disorder and evaluate its influence on the course of myocardial infarction. Of the 292 patients without known diabetes mellitus (DM) type 2 — 31.8 % had prediabetes, and 11.0 % had newly diagnosed DM type 2. Patients with carbohydrate metabolism disorder more often had acute heart failure (AHF Killip II-IV) (40.6 % of patients with normal glycaemia, 55.9 % with prediabetes, 62.6 % and 64.7 % with newly diagnosed and known DM, р = 0.039), and more apparent chronic heart failure (NYHA III-IV) (in 4.2; 9.4; 16.0 and 24.6 % respectively, р < 0.001). Twenty four-hour mortality was higher in patients with known DM type 2 than those without DM (13.2 and 2.3 %, р = 0.008). Regression analysis revealed that impaired glucose metabolism was associated with AHF (Killip II—III (OR = 1.27; 95 % CI: 1.05—1,.54, р = 0.014), Killip IV (OR = 1.39; 95 % CI: 1.06—1.82, р = 0.016)); DM type 2 was associated with higher rate of in-hospital mortality (OR = 4.71; 95 % CI: 2.27—9.78, р < 0.001).

ETIOLOGY AND CLINICAL MANIFESTATIONS OF INTESTINE INTUSSUSCEPTION IN CHILDREN OF ARKHANGELSK REGION

Abstract

A study of manifestations of etiological, age and morbidity aspects of intussusception has been carried out in Arkhangelsk and the Arkhangelsk region in different periods of time of observation and development of the disease’s model in the region. A retrospective analysis of 270 cases of IS in 264 children treated in the Arkhangelsk Children's Hospital from 1980 to 2011 has shown that age and etiological aspects and clinical manifestations of the disease changed with time. The average age of patients increased in the last period of 12.5 (7.0; 30.0) months; Me (Q1; Q3). IS often develops in autumn months (p = 0.001) and in more than a half of the cases - in connection with other diseases. The classical «triad» of the symptoms including abdominal pain, abdominal intussusception and blood from the rectum is detected less frequently, currently in 34.5 % of cases, more often, there is monitored the «diad» of the symptoms (vomiting and abdominal pains) (p < 0.001). Conclusion: the etiology, pathogenesis and clinical manifestations of intussusception have changed with time and have their own characteristics in a particular region. Creating a model of the disease will improve quality of intussusception diagnosis.

DIAGNOSTIC SIGNIFICANCE OF HAND THERMOGRAPHY, ULTRASONIC RESEARCH OF CAROTID AND ARTERIAL PRESSURE IN PATIENTS WITH ISCHEMIC HEART DISEASE

Abstract

A hand thermal imaging study, ultrasonography of the carotid arteries have been carried out and blood pressure has been measured in 31 patients with the ischemic heart disease and in 31 patients with the left scalenus anterior syndrome. It has been established that a decrease in temperature of skin of the forearm lower third, fingers of the left hand not less than by ∆Т 1.1 °С in comparison with symmetric sites of the right hand according to an infra-red thermography, thickening of a layer of the carotids’ “intima-media” by 1 mm and more shown by ultrasonic research, an increase of systolic arterial pressure up to 140 mm Hg and higher were a basis for profound inspection concerning the coronary heart disease.

SYSTEM SYNTHESIS OF MENTAL MEDICINE

Abstract

In the article, efficiency of the synergetic approach to development of mental ecology and mental medicine has been grounded on the conceptual-methodological level. It has been shown that quality of life and a health level are 20 % predetermined by the mentality factor. There has been proposed the «Index of personality mental ecology» allowing to take qualitative and quantitative measurements of its biopsychosocial and spiritual-moral potential and to assess and predict a destiny course. There have been systematized the issues of the crisis in psychiatry and the significance of antipsychiatry for a startup of the contemporary Era of Psychotherapy that has made a revolutionary breakthrough to the prenosological field of public health and public conscience and changed radically the focus of the Ψ-mission of the state and the society from mental illnesses to mental health. That has provided dynamic development of the whole complex of neurosciences, forced establishment of social psychiatry and narcology and lead to birth of mental ecology and mental medicine. Separation of mental medicine as a unified theory of mental health promotion and treatment of mental illnesses has been grounded.

CEREBRAL HEMODYNAMICS IN CHILDREN WITH ATTENTION DEFICIT HYPERACTIVITY DISORDER

Abstract

The results of the study of cerebral circulation. Rheoen-cephalography method was used. The study involved 99 boys of midchildhood with disorder of attention and hyperactivity disorder and 89 children without pathology. It was found that hyperactive children with attention deficit have decreased cerebral blood flow in the carotid system; reduced elastic properties of vessels of large and medium caliber, increased microcirculatory bloodstream vessels tone in the carotid and in the vertebrobasilar brain basins, impaired venous drainage.

Abstract

During a year (winter, spring, summer and autumn) the spirographic survey of one and the same group of healthy children (33 boys and 32 girls of primary school age, natives of Arkhangelsk) was conducted. It is established that in the period of transition from warm to cold season (autumn) children of 8-9 years old have increase in system functional capacities and the level of respiratory breathing. Reduction of IRV and increase of ERV in absolute indicators in comparison with proper values, as well as in relative indicators in relation to VC in all seasons of year was revealed.

Abstract

The article presents the characteristics of heart rate variability among students with different levels of trait anxiety. It is shown that high and medium level of trait anxiety is typical for the majority of the students and is accompanied by increased activity of the sympathetic division of the ANS. Variety of opportunities and strategies of corrective influence of speleoclimatotherapy on the performance of vegetative homeostasis is revealed according to the level of trait anxiety. It is shown that a moderate level of anxiety provides the most efficient adaptive effect of speleoclimatotherapy; and method capacity is limited in individuals with high level of anxiety.
